It's a little difficult to keep control of your emotions when you are trying to purchase a new ultrasound. Within the past ten years, ultrasound machines have improved tremendously in terms of ultrasound imaging resolution, digital capabilities and user-friendliness. We are now able to clearly visualize small structures that we could not visualize ten years ago. That is primarily because computers and ultrasound machines have followed a similar trend in terms of decreasing in size while increasing in power. That is partly due to the fact that an ultrasound system is a computer. It is a computer in which the only software it can run is its respective ultrasound system software.

Which brings us to the question, what's important in determining what I need in an ultrasound machine? Should I believe what my ultrasound rep tells me? How can I confirm what my sales rep tells me? What questions do I need to make sure I ask?

What I can tell you is that every ultrasound equipment manufacturer has its pros and cons. Some are better in Renal Imaging while others are better in Thyroid Imaging. Some have better workflow features in Cardiac, while others are better in Vascular, etc... Therefore, in determining what you need in an ultrasound system, I would start by asking the physicians' who teach their respective hands on ultrasound workshops. I would approach and ask them:

• Which ultrasound machine do they use?

• Why did they choose that system?

• Was their decision based on price or performance? (This will tell you if they were just looking for the cheapest in the market or if they were looking for a quality system that can grow with them)

• How long have they had their system?

• Is there anything they wished was improved on their system?

• Which features are important to them?

• Are they happy with their decision?

Pay close attention to their answers because it will give you a strong insight on which ultrasound system is stronger in your specialty. Once you have that information, you will be ready for your on-site ultrasound demonstration with your sales rep.

During your on-site ultrasound demonstration, your sales rep or sonographer will come in and show you the bells and whistles of their system. They will tell you that their system is better than the competition and probably make a compelling argument. But how do you know if they are telling the truth? I would start by testing their knowledge and credibility. With a little pre-emptive research, you can ask a couple questions you already know the answers to. For example, you can ask the following questions:

• In my specialty, is it required for me to be certified to do ultrasounds? If so, what is the certification process?

• What are the CPT Codes for the procedures I will be billing? What does each CPT code mean? What is my medicare re-imbursement for those codes?

• What additional hands-on workshops can he recommend for my specialty? Is he aware of any physicians' that put on one on one ultrasound training at their facility?

This is just a small sample list of questions that will tell you how knowledgeable and credible the rep truly is. If the rep is untruthful in any of his answers, then you probably have good reason not to believe anything that has been said.

Another way to confirm the truth in all his statements is to ask for 3 or 4 referrals. It's important for you to call those referrals and ask them the same questions that you had asked the person who taught the hands on ultrasound workshops.

In conclusion, the most important question that is usually not asked is regarding warranties. It is good to confirm what exactly is covered under the warranty. For example, there are some that don't cover certain transducers, peripherals (such as an ultrasound printer), or accidental damage to the ultrasound equipment. It's always good to confirm what is and what is not covered under the warranty. There are different types of service warranties offered in the marketplace. For example, there are:

• Depot warranties are the ones where you have to ship your equipment off to a service center. Sometimes it includes a loaner that they ship to you while your equipment gets fixed, sometimes they do not. If a loaner is not part of the warranty, then you better be prepared for some headaches.

• Remote or Online warranties are the ones in which they do remote diagnostics on your ultrasound equipment through the internet.

• Onsite warranties are the ones in which a field service engineer comes on-site to fix your equipment. Call me crazy, but if I am going to spend $30,000 on a new ultrasound equipment, I would want someone to come on-site and fix my ultrasound equipment.

The costs can be around $2000-4000 per year depending on which University to go to. Due to long waiting lists, you may not find entry easily. The only well-known university offering a Post Diploma Ultrasound program in Ontario is Michener Institute.

So what are your alternatives?

2) The other way, and the one that I suggest is learning Ultrasound from an instructor who has experience teaching Ultrasound but is not necessarily working as part of an Institute. Why do I suggest this? Don't you have to go to a certified institution? The answer, plain and simple is NO.

As long as you have the right skills, many ultrasound clinics will hire you as a "trainee-ultrasonogist" without requiring any special certification. After working with such clinics for about a year, you will be eligible for writing your ARDMS examination and becoming a licensed ultrasonogist. This process can take as little as 1 year and 4 months of study and as little as $1500 in investment.

Where do I find such an Instructor?

The one that I personally recommend if you are looking for Ultrasound Technician Schools near Toronto is the Mainz Ultrasound Training (located in Mississauga).

UPDATE: Mainz Ultrasound has recently changed their prerequisites to only accept students who already have some ultrasound background.

Ultrasound Tranducers for Use With Ultrasound Machines


When imaging the body with ultrasound, ultrasound transducers are a key component of the imaging process. Without a transducer, the image cannot be obtained.

There are many types of ultrasound transducers and they vary in size and shape and depending on what part of the body that is being imaged will dictate what type of transducer will be used.

If you are having an abdominal ultrasound, most likely a curvilinear transducer will be used with a frequency of anywhere from 2.0 megahertz to 7.5 megahertz.

If you are having an ultrasound of your thyroid, most likely the transducer will have a frequency of 10 megahertz because the thyroid is not a deep structure and therefore will need a high frequency transducer.

If you are having an early pregnancy ultrasound, most likely a transvaginal ultrasound transducer will be used. This type of transducer is inserted into the vagina approximately 1 - 2 inches without a full bladder and gives the best access to an early pregnancy.

If you are having an ultrasound for pregnancy after 9 weeks gestation, than a curvilinear transducer maybe used with a frequency of 3.5 - 5 megahertz.

Ultrasound transducers will be used according to the study and how deep the tissue is that is being visualized. Most of them are adjustable during the scan but are limited within the device themselves. In other words, one transducer may have a range of 3.5 to 5.0 megahertz but it is not often that a transducer will have a range of 2.0 to 10 megahertz.

Diagnostic ultrasound machines must have transducers in order to see an image. An image cannot be obtained without gel on the skin. The gel works as a coupling to help with image visualization. It is always water based and will not stain your clothing.

The transvaginal probe is used with a condom or probe cover and gel is also applied to the tip of the probe both inside the cover and outside prior to being inserted. Again without the gel, the image will not be obtained.

It is not up to you to worry about what kind of ultrasound machine is used or the transducer used for your exam, the ultrasound tech will know exactly what to do and how to use it effectively.

Make sure that when you are going to have an ultrasound study that you listen carefully and adhere to any preparation instructions that are given to you.

It is important that you are prepared for the test you are having or your test maybe rescheduled. There is a reason for any kind of prep that they tell you to do even though you may think that you don't need it, leave it to the professionals to know what is best for you ultrasound exam.

For example:

Imaging of the gallbladder is done through an abdominal ultrasound; the gallbladder itself shrinks when you eat or drink and therefore cannot be seen by ultrasound. It is therefore recommended that you stay NPO for 12 hours prior to your exam, which means nothing to eat or drink for 12 hours before your ultrasound.

Other preps maybe to fill your bladder prior to having a pelvic ultrasound. This is up to the clinic performing the test. Some of them still have patients drink lots of water and of course hold it while others that feel that filling the bladder does not enhance the visibility and so it is not necessary.

Diseases of the upper aero-digestive tract include tonsillitis, snoring (obstruction and vibration caused by fleshy structures in the back of the throat), swallowing disorders of the esophagus (fistulas), and voice changes (paralysis or pathologic movement of the vocal cords).
